Anthony Sampson considers the stark contrast between the standard of living and consumer spending in the West and the abject poverty in the Third World, particularly in the Ivory Coast of West Africa where people scrabbling for survival on rubbish tips and dying through inability to pay for medical treatment live not far from the tourist retreats of the wealthy.
